Job summary

Novant Health is seeking a BSW Case Manager to facilitate complex discharge planning and coordinate care with the healthcare team. You will collaborate with physicians and RN/MSW for guidance on complex cases and ensure safe, proactive discharge plans aligned with Novant Health policies.

You will act as a discharge planning and care coordination resource, maintaining knowledge of community resources and post-acute options to support patients and families throughout transitions of care.

Qualifications

• Bachelor's degree in Social Work from CSWE-accredited program.
• Two+ years professional social work or healthcare experience.
• NC state approved curricula for restrictive interventions within 3 months of hire.
• Weekend and holiday coverage required.
• Knowledge of Microsoft Office preferred.

Responsibilities

• Coordinate discharge planning and care coordination with the healthcare team.
• Collaborate with physicians to assess, plan, implement and evaluate proactive discharge plans.
• Serve as a resource for discharge planning and post-acute care options.
• Maintain current knowledge of community resources and post-acute options.
