Job summary

Novant Health Matthews Medical Center is seeking a Part-time Case Manager MSW to lead the complex discharge planning process and collaborate with physicians and the health care team. This weekend-friendly role offers 24 hours per week with incentive pay and a pathway to part-time benefits.

The ideal candidate holds a Master’s in Social Work, CSWE-accredited, with 2+ years in social work or healthcare. You will support a collaborative team and contribute to high-quality, safe discharge planning

Qualifications

• Master's Degree in Social Work from a CSWE-accredited program.
• Two or more years of professional social work experience or healthcare experience preferred.
• Holiday coverage required.

Responsibilities

• Lead the complex discharge planning process as a key member of the care team.
• Collaborate with physicians and health professionals to assess, plan, implement, and evaluate discharge plans.
• Coordinate with RN Case Managers for guidance on complex cases.
• Serve as a resource for discharge planning and care coordination for psychosocially complex cases.
• Ensure care coordination aligns with policies and Novant Health values.

You'll work with a wide variety of patient populations, supporting multiple units. This part-time weekender position offers you the perk of working 24 hours each week and earning 8 hours of incentive pay. A weekender position offers you work/life balance while still giving you part-time benefits. must be able to orient full time Monday - Friday for a minimum of 4 weeks prior to moving to weekend only training.

What You'll Do

• Serves as a leader of the complex discharge planning process and a valued member of the healthcare team
• Collaborate with physicians and other members of the healthcare team to assess, plan, implement, and evaluate proactive, complex and safe discharge plans
• Collaborate with RN Case Managers for guidance with medically complex cases
• Serve as a resource for discharge planning and care coordination for psychosocially complex cases
• Responsible for facilitating coordination of care
• Maintain current knowledge of available community resources and post acute care options
• Accepts responsibility, authority and accountability for management of care in accordance with the current policies and procedures, mission, vision, and values of Novant Health

What We're Looking For

• Education: Master's Degree in Social Work from school accredited by the Council on Social Work Education, required.
• Experience: Two or more years professional social work experience or two or more years healthcare experience, strongly preferred.
