Qualifications

• Master's degree in Social Work from an CSWE-accredited program is required.
• Two or more years of professional social work experience or healthcare experience preferred.
• Licensure/certification in accordance with state requirements within 3 months of hire for ED/Behavioral Health.

Responsibilities

• Lead patient assessment and complex discharge planning with the care team.
• Coordinate with physicians and other team members to implement proactive, safe care plans.
• Serve as a resource for discharge planning and care coordination for complex cases.
• Ensure compliance with Novant Health policies and quality metrics.
