Job summary

Qualifications

• Master of Social Work (MSW) degree from an accredited program.
• Indiana Social Work licensure is required.

Responsibilities

• Discharge screening, evaluation and plan implementation for complex needs.
• Reassessment of discharge plans and condition monitoring.
• Provide patient advocacy with healthcare team and support persons.
• Intervene in family stress and emotional needs related to healthcare decisions.
• Crisis intervention and adjustment to illness or bereavement.
• Resource brokering for SNF, home care, hospice, infusion, LTAC, DME, etc.
• Coordinate system integration and interdisciplinary collaboration.
• Support social determinants of health assessments and access to programs.
• Provide advance directives information and execution.
