Job summary

Qualifications

• Master of Social Work from an accredited program.
• Healthcare emphasis or hospital social service experience preferred.
• Behavioral health experience preferred.

Responsibilities

• Discharge screening, evaluation, and develop/implement discharge plans for complex needs.
• Reassess discharge plans and monitor changes in condition.
• Advocate for patients with support persons and healthcare team.
• Provide crisis intervention and support related to illness and adjustment.
• Broker resources for SNF, home care, hospice, and other services.
• Coordinate care transitions and ensure safe, timely discharge.
