Job summary

Medical University of South Carolina (MUSC Health) is seeking a Master’s Social Worker in Case Management to support patients and families during challenging medical situations. You will navigate emotional, social, and practical aspects while coordinating services to improve quality of life.

You’ll interview clients, develop individualized care plans, provide crisis intervention, and collaborate with interdisciplinary teams to ensure seamless care. 40-hour work week, day shift, on-site role.

Qualifications

• Master’s degree in social work from an accredited program.
• LSW or LCSW licensure in South Carolina is required.
• BLS certification from AHA is required.

Responsibilities

• Interview clients and families to assess social and emotional needs.
• Coordinate and plan individualized programs and activities.
• Provide crisis intervention and emotional support.
• Help families understand medical conditions and impacts.
• Collaborate with interdisciplinary teams to ensure seamless care.
• Apply clinical judgment to evolving client needs.
