Job summary

Endeavor Health in Arlington Heights is seeking a Care Coordinator-MSW to support patients with chronic illness within a multidisciplinary team. The role emphasizes care coordination, education, and patient-centered planning.

You will work to ensure safe transitions of care, adherence to evidence-based protocols, and effective collaboration with physicians, nursing, and case managers across departments. This role requires an MSW and IL licensure with patient-family engagement.

Qualifications

• Master's Degree in Social Work and IL LSW/LCSW licensure.
• 1-2 years of social work experience in an acute care setting preferred.
• Illinois licensure as LSW or LCSW; LCSW preferred.

Responsibilities

• Collaborates with multidisciplinary team to support patient populations with chronic illness and multiple co-morbidities.
• Adheres to care management standards with evidence-based protocols and cost-effective interventions.
• Coordinates care across departments and community resources to ensure smooth transitions and optimize total cost of care.
• Applies social work methods to assessment, education, collaboration, and resource coordination to improve patient self-efficacy and quality of life.
• Provides intervention for Endeavor patients and value-based contract members, including care coordination and counseling.
