Job summary

Endeavor Health is seeking a Care Coordinator-MSW in Highland Park, IL. This part-time role offers 20 hours weekly with daytime hours and occasional weekend/holiday coverage. You will provide patient-centered care, coordinate services, and advocate for patients within a collaborative care team.

The ideal candidate has a Master’s in Social Work, LSW/LCSW license, CPR/BLS, and 2+ years of relevant experience. EMR and MS Office skills are preferred.

Qualifications

• Master's Degree in social work from an accredited program.
• LSW or LCSW license required.
• CPR/BLS certification through American Heart Association.
• Two years' post-graduate healthcare or mental health experience preferred.
• Experience with an EMR platform and MS Office preferred.

Responsibilities

• Provide comprehensive direct patient continuum of care and case management.
• Coordinate and refer to health and social welfare resources and discharge planning with care team.
• Function as an integral member of a collaborative interdisciplinary team.
• Support nursing and other clinical/non-clinical services to maintain care quality.
• Act as patient advocate throughout care processes.
