Job summary

Edward Elmhurst Health is seeking a Care Coordinator-MSW in Arlington Heights. The role is full-time (32 hours/week) with Monday–Friday hours and rotating weekends/holidays, to support patients with chronic illness through multidisciplinary collaboration.

The position requires a Master’s in Social Work and Illinois LSW/LCSW licensure; 1–2 years in acute care is preferred. Excellent communication and teamwork are essential for effective care coordination and resource navigation.

Qualifications

• Master’s Degree in Social Work required.
• Illinois licensure as LSW or LCSW; LCSW preferred.
• 1–2 years of social work experience in acute care preferred.

Responsibilities

• Collaborates with the multidisciplinary team to support patients with chronic illness and comorbidities.
• Provides care coordination, education, and resource linkage to ensure safe transitions of care.
• Applies social work methods to improve self-efficacy, health outcomes, and quality of life.
