Job summary

Capital Health in Hopewell is seeking a licensed Master’s of Social Work to assess, intervene, and plan appropriate discharge based on a patient’s clinical, social, and functional needs. You will integrate cultural, ethnic and age considerations into the care plan as an active member of the discharge planning team.

Requirements include an MSW, New Jersey Social Worker license, and at least one year of hospital case management experience.

Qualifications

• Master’s degree in Social Work (MSW) is required.
• New Jersey Social Worker license is required.
• At least 1 year of hospital case management or healthcare experience.
• Licensed Social Worker credential is required.

Responsibilities

• Documents in the clinical record, comprehensive assessment of patient needs and barriers to discharge.
• Identifies barriers and plans interventions to support timely discharge.
• Collaborates with health care team to address social and financial barriers delaying transitions.
• Documents referrals and interventions related to abuse, neglect, domestic violence and other issues.
• Performs mental health assessments and advocates for post-acute services as needed.
• Participates in staff development and performance improvement activities.
