Job summary

Endeavor Health in Evanston, IL is seeking a Care Coordinator MSW to provide comprehensive case management and direct patient care. The role involves coordinating referrals, discharge planning, and collaborating with a multidisciplinary team to maintain high-quality service delivery.

The ideal candidate will hold an MSW with LSW/LCSW licensure, CPR BLS certification, and at least two years of healthcare or mental health experience, with EMR and MS Office proficiency.

Qualifications

• LSW or LCSW license required.
• Master's Degree from an accredited school of social work required.
• CPR BLS Certification required (American Heart Association).
• Two years' post-graduate healthcare and/or mental health experience preferred.
• Experience with Electronic Medical Record (EMR) platform and MS Office preferred.

Responsibilities

• Provide comprehensive direct patient continuum of care and case management services.
• Coordinate referrals to health and social welfare resources and discharge planning with the Continuum Care Manager or Care Navigator.
• Function as an integral member of a collaborative, interdisciplinary team.
• Support nursing services and other clinical and non-clinical adjuncts to ensure continuum of care and quality of service delivery.
• Serve as a patient advocate.

Hourly Pay Range:

$30.46 - $45.69 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights:

• Position: Care Coordinator MSW
• Location: Evanston, IL
• Full Time: 40 hours/week
• Hours: Monday-Friday, 8:30a-5.00p, with weekend rotation every 6-8 weeks
• Coverage for 1 major winter and summer holiday required

What you will do:

• Provide comprehensive direct patient continuum of care, case management services
• Coordination and referral to health and social welfare resources and discharge planning in conjunction with the Continuum Care Manager or Care Navigator
• Functions as integral member of a collaborative and interdisciplinary team
• Supports Nursing services and other clinical and non-clinical ancillary services in assuring the continuum of care for patients and in maintaining the quality of service delivery
• Will also serve as patient advocate

What you will need:

• License: LSW or LCSW license required
• Education: Master's Degree from an accredited school of social work required
• Certification: CPR BLSCertification required-American Heart Association (AHA)
• Experience: Two years' post-graduate experience preferred; healthcare and/or mental health experience preferred
• Skills: Experience with Electronic Medical Record (EMR) platform preferred. Experience with MS Office.
