Endeavor Health is seeking a Part-Time Care Coordinator-MSW to join our Highland Park, IL team. The role focuses on direct patient care, case management, and discharge planning in collaboration with a multidisciplinary team.
Requires Master’s in Social Work and a current LSW or LCSW license, plus CPR/BLS certification. Two years of post-graduate experience preferred, with EMR and MS Office familiarity. Weekend rotation and holiday coverage are required.
Hourly Pay Range: $30.46 - $45.69 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Hours: Monday-Friday, 8:30a-5:00p with rotating weekends and 1 winter and 1 summer holiday coverage required
