Job summary

Endeavor Health is seeking a Part-Time Care Coordinator-MSW to join our Highland Park, IL team. The role focuses on direct patient care, case management, and discharge planning in collaboration with a multidisciplinary team.

Requires Master’s in Social Work and a current LSW or LCSW license, plus CPR/BLS certification. Two years of post-graduate experience preferred, with EMR and MS Office familiarity. Weekend rotation and holiday coverage are required.

• MSW required at accredited school; LSW or LCSW license required.
• Two years post-graduate experience preferred; healthcare/mental health background preferred.
• CPR BLS certification required (AHA).

Responsibilities

• Provide comprehensive direct patient continuum of care, case management services.
• Coordinate and refer to health and social welfare resources and discharge planning with care team.
• Function as integral member of interdisciplinary team; support nursing and other services.
• Act as patient advocate to assure continuum of care and quality of service delivery.

Hourly Pay Range: $30.46 - $45.69 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights

Position: Care Coordinator-MSW

Location: Highland Park, IL

Part Time: 20 hours/week

Hours: Monday-Friday, 8:30a-5:00p with rotating weekends and 1 winter and 1 summer holiday coverage required

What you will do:

• Provide comprehensive direct patient continuum of care, case management services
• Coordination and referral to health and social welfare resources and discharge planning in conjunction with the Continuum Care Manager or Care Navigator
• Functions as integral member of a collaborative and interdisciplinary team
• Supports Nursing services and other clinical and non-clinical ancillary services in assuring the continuum of care for patients and in maintaining the quality of service delivery
• Will also serve as patient advocate

What you will need:

• License: LSW or LCSW license required
• Education: Master’s Degree from an accredited school of social work required
• Certification: CPR BLS Certification required-American Heart Association (AHA)
• Experience: Two years’ post‑graduate experience preferred; healthcare and/or mental health experience preferred
• Skills: Experience with Electronic Medical Record (EMR) platform preferred. Experience with MS Office.
