Qualifications

• MSW required and compliance with state continuing education rules.

Responsibilities

• Performs timely admission screening and interviews to assess social needs.
• Provides social work services and conducts individual/group sessions.
• Directs families to community resources for financial aid and longer-term care.
• Manages complex discharge planning and placement arrangements.
• Arranges ambulance transportation and participates in discharge rounds.
• Documents assessments and interventions in medical records promptly.
• Participates in quality improvement projects and resource transfer data.
• Maintains knowledge of community resources and reports concerns.
