Job summary

HealthPartners in Minnesota is seeking an MSHO/MSC+ Care Coordinator to support patients, families, and physicians in addressing medical and social needs. You will educate and empower patients to make informed health care decisions and facilitate communication among patients, providers, health plans, and community resources.

Qualifications

• Must hold current unrestricted RN license in Minnesota.
• MnCHOICES Assessor Certification in place or within 3 months of hire date.
• Minimum of 2 years home and community based experience; knowledge of utilization review, discharge planning, or case management.

Responsibilities

• Provide member-focused, personalized care coordination for MSHO/MSC+ members.
• Coordinate across patient, family, physicians, health plan, and community resources.
• Educate members on health care decisions and plan benefits; document interactions.
• Maintain confidentiality and accurate case management records.
• Collaborate with Supervisor and departments to ensure Medicare compliance and program effectiveness.
