Qualifications

• This role performs nursing assessments, planning, and evaluation in accordance with hospital policy and professional nursing standards.
• Documents and communicates patient data and care plans clearly to the care team and families.
• Maintains documentation and ensures timely completion of records as required.

Responsibilities

• Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs.
• Formulates a plan of care, including discharge planning, with realistic goals in collaboration with patient and family.
• Implements the plan of care through direct patient care, coordination and supervision of the care team.
