Job summary

ReNew is seeking an Advanced Practice Provider – Mobile Wound Care (Physician Assistant) to evaluate, manage, and provide longitudinal care for complex chronic wounds in patients' homes, focusing on venous stasis, arterial, and pressure injuries.

The PA works as a field-based extension of the supervising Interventional Radiologist, performing in-home wound assessments, coordinating with home health agencies, and supporting physician-directed treatment plans to improve outcomes.

Qualifications

• Licensed Physician Assistant
• Experience in wound care, vascular disease, geriatrics, or chronic disease management preferred
• Comfort with independent home visits within a physician-directed framework
• Strong communication and coordination skills

Responsibilities

• Perform in-home evaluations of chronic and complex wounds
• Obtain targeted histories relevant to wound etiology, vascular disease, diabetes, immobility, and infection risk
• Conduct comprehensive wound assessments, including: size, depth, tissue characteristics, exudate, odor, and surrounding skin
• Determine primary wound etiology: venous stasis, arterial/ischemic, pressure-related, mixed etiologies
• Coordinate care with the Interventional Radiologist and home health agencies under physician-directed plans
• Develop and adjust wound care plans under physician guidance and monitor progression
• Identify patients who may require vascular imaging or intervention or escalation of care
• Participate in weekly or bi-weekly follow-up visits as needed
