Personic Health is seeking a Nurse Practitioner with a strong procedural focus for in-home wound care across Nassau County, NY. The role involves mobile, on-site patient visits and hands-on wound procedures, biopsies, debridement, and injections as needed.

The NP will assess and manage acute and chronic wounds, educate patients, and collaborate with physicians to ensure high-quality care delivered in patients' homes. A NY NP license and ongoing education are required.

Qualifications

• Master's or Doctorate in Nursing required; NP licensure in NY.
• Wound care specialty with hands-on procedural experience desired.
• Strong communication and collaboration with multidisciplinary teams.

Responsabilités

• Perform bedside wound care procedures and advanced wound management.
• Travel to patient homes and facilities as part of a mobile practice.
