Job summary

Healing Partners seeks a Nurse Practitioner to provide home-based wound care for patients transitioning from Skilled Nursing Facilities to home care.

You will perform in-home assessments, develop wound care plans, coordinate with care teams, and travel frequently within your territory to deliver high-quality care. A strong focus on improving healing outcomes and reducing rehospitalizations is expected.

Qualifications

• MSN or DNP or equivalent required.
• Active NP license and RN license in practicing states.
• National board certification as Nurse Practitioner.
• Current BLS certification.

Responsibilities

• Conduct in-home wound assessments for patients transitioning from SNFs to home.
• Develop and manage evidence-based wound care treatment plans.
• Coordinate care with primary care providers, specialists, home health agencies, caregivers, and referral partners.
• Travel daily within assigned territory to conduct patient visits.
