Job summary

Dermacore Wound Care seeks a Nurse Practitioner for a mobile wound care role on a per diem basis. You will perform comprehensive wound assessments, develop individualized treatment plans, and conduct wound care visits in patients’ homes and community settings, coordinating with an interdisciplinary team.

Candidates should hold an NP license with national certification and be capable of independent field practice while maintaining strong communication and documentation with the care team.

Qualifications

• Current and unrestricted NP license in the applicable state.
• Current national certification with prescribing authority, as applicable.
• Wound care experience preferred; additional wound care education may be provided based on experience.

Responsibilities

• Perform comprehensive patient and wound assessments and develop individualized treatment plans.
• Provide wound care visits based on patient’s clinical needs and established plan of care.
• Perform wound care procedures within scope of practice, training, and clinical privileges.
• Develop, update, and manage wound-related treatment recommendations and orders.
• Monitor wound progression and adjust treatment plans when clinically indicated.
• Complete accurate clinical documentation within required timelines, with same-day charting.
