Job summary

AllPeacewoundcare is seeking a Mobile Wound Care Nurse Practitioner to deliver expert care across skilled nursing facilities, assisted living communities, and home settings. You will lead comprehensive wound assessments, create individualized treatment plans, and work independently with support from an interdisciplinary team.

The role emphasizes flexible scheduling, meaningful patient impact, and opportunities for continuing education within a supportive clinical team.

Qualifications

• Active and unrestricted Nurse Practitioner license.
• DEA registration or eligibility.
• Strong clinical assessment and decision-making skills.
• Excellent communication and documentation abilities.
• Valid driver’s license and reliable transportation.
• Wound care experience or certification preferred, but motivated candidates are encouraged to apply.

Responsibilities

• Conduct comprehensive wound assessments.
• Develop and implement individualized treatment plans.
• Manage acute and chronic wounds, including pressure injuries, diabetic ulcers, surgical wounds, and other complex wounds.
• Perform wound-related procedures within scope of practice.
• Document patient encounters accurately and promptly.
• Collaborate with physicians, facility staff, caregivers, and interdisciplinary teams.
• Educate patients and caregivers on wound prevention and treatment.
