Qualifications

• Graduate of an accredited NP or PA program.
• Current, unrestricted APRN/PA license in Oklahoma.
• Current national board certification.
• Valid driver's license and ability to travel within the service area.
• Strong clinical assessment and decision-making skills.
• Ability to work independently with a mobile patient schedule.
• Excellent communication, organization, and documentation skills.
• Two or more years of advanced practice experience preferred.
• Wound care certification preferred; may need to obtain within six months of hire.

Responsibilities

• Perform comprehensive patient and wound assessments.
• Develop and manage individualized wound care treatment plans.
• Assess, measure, stage, and document wounds and wound characteristics.
• Perform wound care procedures within scope of practice, including debridement as appropriate.
• Utilize advanced wound care products, therapies, and modalities.
• Assess vascular status and perform vascular screening, including ABI/TBI when indicated.
• Identify barriers to wound healing and coordinate referrals and tests.
• Collaborate with physicians, nursing staff, home health and facility staff.
• Educate patients, caregivers, and staff on wound care, prevention, nutrition, offloading, and compression.
• Maintain accurate, timely clinical documentation.
• Practice in line with HealWell policies, HIPAA, and regulations.
• Maintain company equipment and wound care supplies.
• Travel to assigned patient locations as required.
