Cleveland Clinic Health System seeks a nationally certified Advanced Practice Provider (NP/PA/CNS) to deliver primary and specialty care in home-based wound Ostomy and continence care across regions. Provides clinical care in collaboration with the health care team, focusing on health promotion and disease prevention.

This PRN role emphasizes education, leadership in developing educational programs, and ensuring continuity of care, with day shifts 8:00am-5:00am in various locations; must hold

Qualificações

• Nationally certified Advanced Practice Provider with wound care experience.
• Wound, Ostomy and Continence Nursing Certification Board certification required.
• Licensed as NP/PA/CNS in applicable state(s); credentialed by Medical Executive Committee.

Responsabilidades

• Take thorough medical histories and perform complete physical exams.
• Order and interpret labs, imaging and other tests to evaluate illness.
• Prescribe and monitor medications per licensure and policy.
• Educate patients on disease, treatment options and side effects.
