Primary Vascular Care is a mobile practice providing on-site vascular assessments for residents in long-term care facilities in the New York City area. The NP/PA will perform comprehensive assessments to determine optimal patient treatment plans and reports to the Lead NP.

Travel throughout the New York City area is required to serve client facilities. Responsibilities include independent history and physicals for complex patients, ordering tests, ensuring regulatory compliance, monitoring

Qualificações

• Bachelor’s degree required.
• Degree from an accredited NP or PA program required.
• Current NP license required.
• BLS certification required.
• ACLS preferred.
• Infection control certification required.
• Board certification or eligibility.
• Vaccines and titers including TB, Hep B, MMR, Varicella required.
• COVID and Flu vaccine preferred.
• 1 year NP experience preferred.

Responsabilidades

• Performs on-site screening clinical assessments to determine appropriate patient treatment plans.
• Independently conducts history and physicals for complex perioperative patients.
• Orders and reviews diagnostic and therapeutic tests per patient needs.
• Assists with care in accordance with facility, state, and federal regulations.
• Monitors the effectiveness of interventions.
• Maintains relationships with facility contacts.
• Educates family members and clients about services offered.
• Assists with physician relations as assigned.
• Completes weekly and monthly reports in a timely manner.

Formação académica

Bachelor’s Degree

Nurse Practitioner or Physician Assistant Program degree

Current NP License

BLS – Basic Life Support (AHA)

ACLS – Advanced Cardiac Life Support (AHA)

Infection Control Certification

Board Certified/Board Eligible

Vaccines and Titers: PPD/TB, Hep B, MMR, Varicella

COVID and Flu Vaccine preferred

1 year NP experience preferred
