Job summary

The role requires CA NP licensure, 2+ years in direct patient care, and experience with chronic disease management. Base salary is $125,400–$172,780 per year, with mileage reimbursement and a strong benefits package.

Qualifications

• Licensed Nurse Practitioner in California. Primary care and field experience preferred.
• Experience with chronic disease management and population health.
• Excellent communication and documentation skills.

Responsibilities

• Perform physical exams, histories, and diagnostic assessments.
• Develop individualized care plans with patients and care team.
• Educate patients on health issues and follow up on chronic conditions.
• Coordinate with physicians, nurse directors, and peer navigators.
