Your Health Organization in Macon, GA is hiring a Speech Therapist to evaluate and treat adults with speech, language, cognitive-communication, and swallowing disorders across home, facility, and outpatient settings. You will join interdisciplinary teams to provide person-centered therapy and monitor progress.

The role is full-time with opportunities for a monthly bonus and a vehicle allowance. Travel across the Macon area is required, with shifts Monday–Friday and times aligning with patient

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Active state license and ASHA Certification (CCC-SLP).
• 1–2 years of experience in adult or geriatric therapy preferred.
• Experience with neurological and cognitive impairments is a plus.
• Strong documentation, communication, and time management skills.
• Willingness to travel across assigned regions as needed.
• Proficient with technology tools for remote patient monitoring and EMR.

Responsabilités

• Conduct comprehensive assessments for speech, language, cognition, and swallowing.
• Develop and implement individualized treatment plans based on patient needs.
• Perform re-evaluations and modify treatment approaches as appropriate.
• Monitor and document patient progress toward goals.
• Collaborate with physicians, nurses, therapists, and care coordinators.
• Educate patients and families on communication strategies and swallowing precautions.
• Coordinate transitions of care and discharge planning when appropriate.
