Job summary

Baptist Health Home Care seeks a Speech Language Pathologist to deliver skilled therapy within our Kentucky home-care territory. You will evaluate swallowing, cognition and speech, develop individualized care plans, and educate patients and families while documenting outcomes for compliant care.

The role requires travel among the listed counties and collaboration with physicians and care teams. A current KY SLP license and driving eligibility are essential.

Qualifications

• A current Speech-Language Pathologist (SLP) license in the state of employment.
• Minimum of one year of experience as an SLP is preferred.
• Home care experience is desired.
• Ability to communicate effectively with patients, families, physicians and co-workers.
• Knowledgeable on speech therapy best practices.
• Ability to make appropriate speech therapy judgments.
• Detail-oriented and observant.
• Disciplined work ethic with the ability to prioritize and be timely.
• Valid driver’s license and auto insurance with your name listed as driver.

Responsibilities

• Provides skilled speech therapy as ordered by the attending physician.
• Evaluates patient functioning scores using diagnostic and prognostic procedures.
• Develops individualized speech-language pathology plan of care and adjusts as needed.
• Records and reports patient progress to physician and agency staff.
• Guides patient and family in plan of care and use of therapies and equipment.
• Promotes continuity of care during admissions, transfers, and discharges.
• Completes documentation in accordance with agency and Medicare guidelines.
