Job summary

Lifeline Home Health, a leading post-acute care partner within the Optum family of UnitedHealth Group, seeks a Speech Therapist in Kentucky to assess and treat speech, language, swallowing and cognitive needs in patients’ homes. You will develop treatment plans, conduct interventions, monitor progress and collaborate with the care team, patients and families to improve communication and overall well-being.

Required: state licensure, valid driver’s license and access to reliable transportation;

Qualifications

• Licensed Speech Therapist in the state of residence.
• Valid driver’s license with access to reliable transportation.
• CPR certification preferred or ability to obtain within 90 days of hire.
• Available for weekend rotation.

Responsibilities

• Provides services within the scope of practice as defined by state laws and care plans.
• Evaluates function and assists in developing the therapy plan of care.
• Treats patients to improve communication, swallowing and cognitive abilities.
• Documents patient progress and communicates with the health care team.
• Educates patient, family and caregivers in speech therapy activities.
• Function in home settings regardless of age, race, creed, or disability.
