Job summary

Hope Home Care, part of the LiveWell Partners Network, is seeking a Speech Language Pathologist (SLP) to evaluate patient needs in the home health setting and develop treatment plans. The SLP collaborates with the care team, physicians, and stakeholders to deliver therapy services and ensure continuity of care.

The role requires active SLP licensure, CPR certification, and a valid driver’s license, with a preference for 2+ years in acute or post-acute care.

Qualifications

• Minimum 2 years of Speech Language Pathologist experience; acute or post-acute setting preferred.
• Active state licensure required for SLP.
• Proof of CPR certification and valid driver’s license required.
• Proof of auto insurance coverage required.

Responsibilities

• Establish patient goals and help patients meet goals by providing quality home care, including evaluating results.
• Determine therapy plans in consultation with care team and physicians.
• Prepare written treatment plans for physician signature, including goals and rehab potential.
• Complete discharge planning with care team and physicians; coordinate post-discharge home exercise programs and follow-up.
• Participate in case conferences and document patient care in records.
• Perform other duties as reasonably assigned.
