Job summary

LiveWell Partners seeks a Speech Language Pathologist (SLP) to evaluate patients in home care, create and oversee treatment plans, and collaborate with an interdisciplinary care team including physicians.

The role requires active state SLP licensure, CPR certification, and a valid driver’s license with auto insurance. The position is PRN with minimum 6 visits per week, serving in the field with mileage reimbursed.

Qualifications

• Minimum 2 years of SLP experience, preferably in acute or post-acute care.
• Active state licensure in Speech Language Pathology is required.
• Proof of CPR certification and valid driver’s license; proof of auto insurance.

Responsibilities

• Establish patient goals and help patients meet their unique goals by providing quality care, including assessing and interpreting evaluations and test results.
• Determine speech therapy treatment plans in consultation with the care team and physicians.
• Prepare written plan of treatment for physician’s signature, including clinical findings, goals, and rehab potential.
• Complete discharge planning by consulting with the care team and physicians and assure continuation of the therapeutic plan post-discharge.
• Participate in case conferences.
• Document patient care services by charting in patient and department records.
• Perform other duties as reasonably assigned.
