Job summary

Brigade Health is seeking a compassionate Mobile Primary Care Nurse Practitioner or Physician Assistant to join our team serving older adults in Santa Rosa, CA. You will deliver primary care in homes and senior living settings, coordinate across disciplines, and build strong resident and caregiver relationships.

The role emphasizes autonomy with support, dementia-informed care, and opportunities for growth within a expanding geri-psych and cognitive health initiatives.

Qualifications

• Active, unencumbered NP or PA license in the practicing state.
• Minimum 1–2 years of clinical experience (geriatrics or primary care preferred).
• Compassionate, patient-centered approach with strong clinical judgment.
• Valid driver’s license and reliable transportation.

Responsibilities

• Deliver personalized, high-quality care to older adults in homes and senior living settings.
• Conduct comprehensive assessments and develop individualized care plans.
• Manage chronic conditions with skill and empathy.
• Collaborate across clinical disciplines (primary, psych, palliative, neuro) for whole-person care.
• Build trusted relationships with residents, caregivers, and community partners.
• Maintain thorough documentation and consistent communication.
