Job summary

Cambridge Healthcare Services, LLC is seeking an Internal Physical Therapy Traveler to provide resident-centered PT services across multiple SNFs within the organization. You will ensure continuity of care, support facility therapy teams, and uphold regulatory standards.

Travel between sites on a regular schedule, document progress in the EMR, and contribute to resident outcomes. Requires CA PT license, knowledge of geriatric rehab, and ability to mentor staff.

Qualifications

• Licensed Physical Therapist in California.
• Experience in geriatric rehab and post-acute care preferred.
• Excellent communication and problem-solving skills.
• Competent in electronic medical records and documentation standards.

Responsibilities

• Provide PT services per physician orders and facility protocols.
• Evaluate residents, develop care plans, and document progress in EMR.
• Deliver individual and group therapy sessions and travel between facilities.
• Collaborate with facility leadership and interdisciplinary teams.
• Mentor and train local therapy staff and participate in QA activities.
