Qualifications

• Must possess a Bachelor’s Degree in Physical Therapy.
• Must be a licensed Physical Therapist in Illinois.
• BLS certification from American Heart Association required upon hire.

Responsibilities

• Provide patient-centered outpatient physical therapy in homes.
• Use telehealth services to support patient care.
• Maintain primary responsibility over patient plans of care.
