Job summary

Your Health Org is seeking a compassionate Primary Care Physical Therapist to serve patients across the Aiken area, performing visits in homes, ALFs, clinics, and via Telehealth. You will conduct comprehensive assessments, provide health education, and coordinate referrals to improve mobility and quality of life.

You will work in a full-time, eight-hour schedule (Mon-Fri) with a monthly guaranteed bonus and opportunities for additional bonuses as part of a salary-based role.

Qualifications

• Successful completion of a Physical Therapy program and state board certification.
• Current SC license to practice.
• Current CPR Certificate.
• Healthcare setting experience preferred.
• Willingness to travel between offices and facilities.

Responsibilities

• Provide Physical Therapy visits in homes, ALFs, ILFs, clinics, and via Telehealth.
• Obtain health and psychosocial history and perform comprehensive physical assessments.
• Provide health education and counseling; coordinate referrals to other health team members.
• Respect patient privacy, obtain consent, and document according to regulations.
• Market and provide community education.
