Your Health Organization is seeking a Primary Care Physical Therapist to serve patients across the Augusta area. The role involves home, assisted living, and clinic visits, with Telehealth options.

This full-time position offers a salary-based pay with a monthly bonus opportunity, and standard 8-hour workdays Monday through Friday. We value compassionate, empathetic clinicians who communicate well and engage with communities.

Qualifications

• Successful completion of a Physical Therapy training program and state board certification.
• Current license to practice in the state (SC).
• Current CPR certificate.

Responsibilities

• Perform home, ALF/ILF, clinic, and Telehealth visits.
• Conduct comprehensive physical assessments and health education.
• Document health history and findings; refer to team members as needed.
• Coordinate with facilities and families to ensure coordinated, compassionate care.
