Brigade Health is seeking a compassionate Nurse Practitioner or Physician Assistant to join our Mobile Primary Care Team, delivering high-quality, home-based care to older adults. You will work across homes, senior living communities, and related settings with a multidisciplinary team to promote wellbeing and independence.

Qualifications

• Active, unencumbered NP or PA license in the practicing state.
• 1–2 years of clinical experience (geriatrics or primary care preferred).
• Compassionate, patient-centered approach with strong clinical judgment.
• Valid driver's license and reliable transportation.

Responsibilities

• Deliver personalized, high-quality care to older adults in homes and senior living settings.
