Access Healthcare Associates seeks a compassionate Nurse Practitioner or Physician Assistant to join our Mobile Primary Care Team, delivering medical care to older adults in homes and senior living communities.

You will perform comprehensive assessments, develop care plans, and manage chronic conditions while collaborating with multidisciplinary teams to provide whole-person care. A current NP/PA license, 1–2 years geriatrics or primary care experience, and a valid driver’s license are required.
