Job summary

Ennoble Care is seeking an experienced Nurse Practitioner or Physician Assistant to provide high-quality primary and palliative care to homebound patients in Seminole County, FL. Work under supervision of a physician to manage a diverse patient population with chronic diseases.

You will perform exams, create care plans, and collaborate across clinical operations. An independent, patient-centered clinician with 1–2 years in primary care is ideal; hospice experience is a plus.

Qualifications

• Board certified NP or PA with Florida license.
• Strong clinical skills to assess, diagnose and manage care.
• Positive demeanor and strong people skills.
• Ability to solve problems independently.
• 1–2 years in primary care with geriatric patients; hospice exp preferred.

Responsibilities

• Provide high quality primary and palliative care in Seminole County, FL.
• Perform physical examinations and develop care plans.
• Communicate with office staff across platforms.
• Deliver patient-centered care to a panel of patients.
• Attend quarterly medical staff meetings.
• Develop impressions, diagnoses and treatment plans.
• Consult with patients, families and caregivers; order tests as needed.
• Prescribe medications and supplies.
