Job summary

Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to provide compassionate, patient-focused care in the home setting across North Carolina. You will evaluate patients, design and implement therapy plans, and educate families while coordinating with physicians and care teams.

This role requires a valid NC driver's license, reliable transportation, and the ability to work under physician-approved plans of care.

Qualifications

• Must be licensed as a Physical Therapist in the state of employment.
• Experience in home health care or outpatient settings preferred.
• Strong communication and collaborative skills.

Responsibilities

• Examine and treat patients with physical impairments using modality-based therapy.
• Improve mobility and daily living activities for clients.
• Deliver skilled care under physician orders and established care plans.
