Job summary

Enhabit Home Health & Hospice is seeking a Physical Therapist to assess and treat patients with mobility challenges who require skilled services in the home setting. The role focuses on improving function, independence in daily activities, and quality of life under physician orders and established plans of care.

Responsibilities include hands-on therapy, patient-family education, and coordination with the health care team under the Branch Director's supervision.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments through physical modalities.
• Improve mobility and independent self-care for daily living.
• Administer skilled care under physician orders and plan of care, supervised by Branch Director.
