Job summary

Enhabit Home Health & Hospice is seeking a Physical Therapist to evaluate and treat patients in the home setting, focusing on improving mobility and independence in daily activities. You will implement specialized programs and collaborate with families and the health care team to achieve goals.

The role requires current Texas licensure, a valid driver's license, auto insurance, and reliable transportation to reach patients in all weather conditions.

Qualifications

• Must hold a valid state driver’s license.
• Must carry automobile liability insurance.
• Must have dependable transportation in good condition.
• Must be licensed to practice in Texas.

Responsibilities

• Assess and treat patients with physical impairments using physical modalities.
• Aim to improve mobility, independence in self-care, and daily living skills.
• Deliver specialty programs: Balance and Fall Prevention, Spine Safety, hip/ knee and shoulder programs.
• Provide skilled care under physician orders and plan of care, with supervision from Branch Director.
