Qualifications

• Licensed Physical Therapist in State of Illinois.
• BLS from American Heart Association upon hire.
• Experience in neuro and ortho PT preferred.

Responsibilities

• Deliver outpatient PT directly in patients’ homes.
• Maintain oversight of patient care plans.
• Participate in departmental process improvement.
