Job summary

Adaptive Home Health is seeking a Physical Therapist to join our Texas home health team, responsible for owning the caseload as a case manager, performing in-home assessments, and delivering skilled PT visits to restore function and promote safety at home.

You will design a plan of care for gait/balance, transfers, strengthening, and home safety, track progress, and coordinate with the care team. Strong independence, communication, and travel readiness are essential.

Qualifications

• Active Texas PT license (in good standing).
• Strong clinical judgment and comfort working independently in the field.
• Comfortable escalating changes in condition to RN/MD and documenting rationale clearly.
• Strong communication and documentation.
• Reliable follow-through on orders, frequencies, and patient goals.
• Current driver’s license and comfort with daily travel across assigned territory.
• Able to carry/transport basic therapy equipment as needed.
• Competency in core home health PT scope: gait training, transfers, strengthening, home safety.

Responsibilities

• Own the caseload as a case manager: therapy plan, progression, outcomes.
• Complete comprehensive in-home assessments and deliver skilled PT visits.
• Build and execute a clear plan of care: gait/balance, transfers, strengthening, mobility, home safety.
• Set measurable goals, track progress, adjust treatment as needed.
• Drive multidisciplinary coordination with care team.
• Manage treatment and PTA supervision where appropriate.
• Coach patients and caregivers with practical home programs.
