Job summary

Adaptive Home Health is seeking a Physical Therapist (PT) to provide skilled in-home therapy across Texas, focusing on gait, balance, transfers, strengthening, and functional mobility.

You will manage patient progress, coordinate with the care team, and document in HCHB to ensure defensible and efficient care. A Texas PT license and daily travel are required.

Qualifications

• Active Texas PT license (in good standing).
• Strong clinical judgment and comfort working independently in the field.
• Comfortable escalating changes in condition to RN/MD and documenting rationale clearly.
• Strong communication and documentation.
• Reliable follow-through on orders, frequencies, and patient goals.
• Current driver’s license and comfort with daily travel across assigned territory.
• Able to carry/transport basic therapy equipment as needed.
• Competency in core home health PT scope.
• Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations.
• Strengthening, functional mobility, and home safety assessment/modification education.
• Patient/caregiver coaching with practical home programs.

Responsibilities

• Complete comprehensive in-home assessments and deliver skilled PT visits.
• Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.
• Set measurable goals, track progress, adjust treatment when needed.
• Drive multidisciplinary coordination and keep the care team moving.
• Manage treatment and outcomes, including PTA supervision where appropriate.
• Coach patients and caregivers with practical home programs.
• Document promptly and thoroughly in Homecare Homebase (HCHB).

Physical Therapist (PT) - Home Health

• Schedule: Full-time Monday–Friday + occasional weekends (as needed)

• Location: Field Based

• Compensation (Productivity Points): $75 per point

What you will do:

• Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.
