BAYADA Home Health Care, Inc. is seeking an experienced Physical Therapist to perform home health visits in the Alexandria, VA area.

This full-time role integrates with a multi-disciplinary team to provide rehabilitative care and skilled nursing in clients' homes. Responsibilities include making home visits, conducting mobility and safety assessments, developing care plans, educating clients and families, and documenting care using EMR.

Formación

• VA PT license required.
• Minimum of 1 year of PT experience.
• Graduate from an APTA/AMA-accredited PT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Experience with EMR systems preferred.

Responsabilidades

• Make home visits to clients in designated geographic territories.
• Assess mobility, gait, balance, strength, and safety.
• Develop and revise individualized care plans.
• Educate clients and caregiver(s) in rehabilitative care.
• Document observations and interventions using EMR.
• Collaborate with nursing as part of the multidisciplinary team.
