Job summary

Caringhand Home Health Solutions, Inc. is seeking a field-based Physical Therapist to deliver skilled care in patients’ homes across California. This per diem position requires travel to patients’ homes, with compensation of $160–$180 per visit.

You will evaluate functional status, educate patients and families, and collaborate with physicians and the care team to ensure high-quality outcomes. A CA PT license, CPR, and a valid driver's license are required.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Current CPR certification with AHA.
• Valid driver’s license and reliable, insured vehicle.

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop treatment plans.
• Deliver PT services per physician’s plan of care and California PT Practice Act.
• Educate patients and families on exercises, equipment use, and recovery goals.
• Collaborate with physicians and care team—reporting changes in patient condition and participating in case conferences.
• Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.
