Job summary

BrightSpring Health Services in Vancouver, WA is seeking a licensed Physical Therapist to join our Home & Community Program on a PRN W2 basis. You will deliver functional PT in home and community settings, helping clients return to work, driving, and community mobility.

You will collaborate with OTs, SLPs, and LCSWs in an interdisciplinary team, with flexible hours and hourly pay. Drive time and mileage reimbursement, educational programs, and clinician-focused support are provided.

Qualifications

Minimum of a bachelor's degree in Physical Therapy from an accredited program.
Current, unrestricted license as Physical Therapist in the state where you practice.
Current Basic Life Support or Cardiopulmonary Resuscitation certification in accordance with state regulations.
Minimum of one year's work experience as a physical therapist preferred.
Demonstrated knowledge of rehabilitation techniques related to complex neurological injury.
Effective and professional verbal and written communication skills.
Ability to lift 50 pounds; lifting up to 100 pounds may be required following safety procedures.
Fine-motor skills, visual acuity, and walking/standing for extended periods.
Ability to push, pull, bend, stoop, reach, kneel and frequently position patients.
Health screen or examination may be required prior to assignment and periodically thereafter, as per local or state regulations, to verify physical capability.

Responsibilities

Perform evaluations and develop treatment plans.
Regularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences.
Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources.
Communicate patient's needs and progress to the treatment team, physician, person receiving services and family members.
Communicate with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes.
