Job summary

Share is seeking an Outreach Care Coordinator to provide client-centered, trauma-informed street outreach to people experiencing homelessness in Clark County. Duties include housing assistance, basic needs provision, referrals for health services, and transportation support to help clients reach stability.

The role emphasizes injury prevention, drug checking, and data reporting, with flexible scheduling and potential evenings/weekends.

Qualifications

• Two (2) years’ experience working with people experiencing homelessness.
• Knowledge of injury prevention practices and ability to provide non-judgmental services.
• Experience with people who use drugs and injury prevention/public health services preferred.
• Experience providing residue drug checking or willingness to complete training.
• Knowledge of infectious disease prevention and overdose prevention practices preferred.
• Experience working with diverse communities.
• CPR, blood borne pathogens, basic first aid certification, naloxone training acceptable.
• Ability to use Microsoft Office and HMIS database.
• Ability to collect, maintain, and report program data for multiple contracts and funding sources.
• Ability to work collaboratively in a team.
• Must have a valid driver's license.

Responsibilities

• Provide comprehensive injury prevention services through mobile outreach and community-based delivery.
• Distribute injury prevention supplies and naloxone per health department requirements.
• Conduct residue drug checking including sample collection, testing, and education on overdose risk reduction.
• Provide accurate, non-judgmental information regarding testing results and health resources.
• Maintain drug checking supplies, equipment, inventory, and QA procedures.
• Maintain documentation and reporting for injury prevention, drug checking, and grant performance measures.
• Identify and implement engagement strategies to reach unsheltered community members.
• Provide trauma-informed outreach and engagement for people experiencing homelessness.
• Assist participants in linkage to housing and supportive services.
• Maintain client engagement records in HMIS and other databases.
• Gather data and feedback to demonstrate program outcomes and submit required reports.
• Respond to crisis situations as needed.
• Engage community members and local businesses to improve engagement with homelessness.
• Attend trainings, meetings, and workshops as required.
• Support policy development and quality improvement initiatives.
