Job summary

Talented-Medical-Solutions in Holyoke, MA, is seeking a Physical Therapist to join a mobile outpatient therapy team serving patients in and around Holyoke. The role offers 30 hours per week, Monday–Friday, with no weekend rotation, and compensation up to $63 per hour based on unit rate.

You will travel between patient locations, develop individualized plans of care, and provide skilled PT interventions to improve strength, balance, mobility and independence. New graduates are welcome to apply.

Qualifications

• DPT or equivalent degree from an accredited program.
• Massachusetts license to practice physical therapy.
• Strong communication and interpersonal skills.
• Ability to work independently while maintaining high clinical standards.
• Organized and adaptable approach to patient care.

Responsibilities

• Develop individualized plans of care.
• Provide skilled physical therapy interventions based on patient needs.
• Help patients improve strength, balance, mobility, endurance and independence.
• Track patient progress and modify treatment plans accordingly.
• Provide patient and caregiver education.
• Maintain timely and accurate documentation.
• Coordinate with other healthcare professionals when appropriate.
• Travel between patient locations as required by the mobile therapy model.
