Qualifications

• Active California professional license and local business license required.
• 1099 independent contractor status; not a Home Health Agency.
• Clinicians must onboard via Suru app and handle own caseload.
• Eligible patients are matched in network with flexible scheduling.

Responsibilities

• Onboard patients through the Suru app/web app.
• Provide PT/OT follow-up visits in patients' homes.
• Accept or decline available cases in your service area.
• Submit visit notes; Suru handles intake, eligibility, and billing.
