• MSN from an accredited program.
• NP certification.
• Active, unrestricted California RN and NP licenses.
• Ability to travel throughout assigned area.

• Provide comprehensive primary care to medically complex patients in patients' homes and via virtual visits.
• Operate with autonomy and in collaboration with an interdisciplinary team to improve patient outcomes.
• Practice at the top of your license and deliver relationship-based care.
